Service Agreement

COST OF SERVICES

The cost of services is based on the package preference such as is listed below, each of which could be up to 10 hours.

IMPORTANT NOTES:

1. By selecting a package or packages and signing this Agreement, the candidate is committing to being on time for all sessions per the schedule within the 10 hours. Additional sessions can be requested as needed at a rate of $50 per hour.

2. We work with the candidate to try and accommodate their schedules as much as possible to set a mutually acceptable timeline for services. Missed deadlines and/or missed appointments may impact the package hours and overall set timeline. It is therefore imperative that the candidate and others involved are engaged to align with the set schedule for a timely execution.

Package 1 - $250 Provides a comprehensive Assessment of the client, this includes interviews, and a self-assessment to identify passions, interests, skills, abilities, conditions, support needed, etc.

Package 2 - $250 Introduces the client to one or two job roles they can excel at based on their interests, passions, skillsets and abilities. During this phase the candidate narrows down their career interests.

Package 3 – $250 Provides the candidate with job shadowing opportunities in their area of interest in an integrated work environment. In some cases this may take the form of a mini internship experience.

 

Add Ons

Add Ons – These are items that ensure Monarch provides a full continuum or employment solutions. The price on these services varies, i.e. is dependent on services needed to close identified employment readiness gaps including but not limited to Job coaching, Social skills strengthening, Executive functioning skills Etc.


PAYMENT OF FEES

Upon signing this Agreement and selecting a package/s of choice, you will receive an electronic invoice which will llustrate an itemized breakout of all services that will be rendered to pay upfront. Acceptable methods of payment include, cash and Venmo. There will be a $35 charge for non-sufficient funds check and rejected electronic payment charges.


CANCELLATIONS

Cancellation by client must be made twenty-four hours in advance of service to avoid charges. Missed/no-show appointments will be charged the regular fee. If a client has an unreasonable amount of late cancellations or missed/no-show appointments it is at Monarch Employment Solutions, LLC discretion to continue services with the client.
